- Requirements:
- EPC (no gap) 1 hour
- - E.P.C. referral,
- - what the focus of education should be e.g. insulin start, general education, help problem solve erratic B.S.L.'s e.t.c.,
- - blood results (N.D.S.S. forms can not be signed without this information) e.g. eGFR, ACR, HbA1c, fasting glucose, OGTT, lipid levels, TFT, vitamin D, c peptide, insulin levels, auto antibodies as relevant
- - up to date medication list (including OTC, complimentary medicines, sport supplements, illicit drug use)
- - ask patient to bring all their medications as it is important to see how they are taking the medication, their blood glucose results from their meter either log book or print out from app/ web page.
- - If they can not speak English they will need to bring someone who can interpret for them (the government does not supply interpreting services for allied health in the community)
- $70 1 hour private
